Registration Notes & Requirements |

Spring 2012 Registration |
Registrations for all Spring 2012 players and coaches/managers is now open online. In person registration will take place at the Brick in Burleson on Jan. 14 and 21 from 10 am-2 pm. Coaches and managers will need to register for the Spring season again, but you can search the database for your background check as long as the name and date of birth are the same without repeating the background check. Please email Julie Mason at moc.reccosasib@nosam.eiluj with any questions. If your player is a new player this season or if your player is returning and needs to turn in a birth certificate, those can be emailed to Julie Mason or you can email Julie Mason to make arrangements to get this copy to BISA. We can only accept typed County Birth Certificates (not hand-written or hospital copies) or passports for verification of date of birth. Thanks so much for your interest in BISA and we look forward to a great season starting in March, 2012.
Players born between August 1, 2008 and December 31, 2008 are eligible for our U4 division this season but are not included in the age chart at the bottom of this page. |

Team Formation Procedures |

Returning Players |
| If you are a returning player wanting to return to your team, be sure to register on time. |
| |

Team Assignments |
| Your coach will receive their team assignments at the coaches meeting, you will hear from them after that date. |
| |
|
Special Requests |
| Parents may request their child to play with a sibling. Parents may also request their child to play up one age level. Players cannot play down an age group. |

Age Verification |
| BISA is required to keep a copy of each players state certified birth certificate (not hospital) or other government issued documentation showing birth date for a player to be added to a team. If the player was born in Texas a copy can be requested from the county courthouse. |

Refund Policy |
Burleson Independent Soccer Association (BISA) is an organization meant to provide an opportunity for Burleson Area youth to play the game of soccer at a recreational level.
The BISA Board and Coaches have absolute control (within the extent of scheduling restrictions) as to selection of practice times, locations, and days. Youth are typically assigned to teams, uniforms purchased, and field insurance secured weeks in advance of practice and game field scheduling. As such, once a child is registered, refunds will be at the sole discretion of the Board. There are NO REFUNDS for practice schedule conflicts, disagreement with coaching staff, a decision not to play with BISA, or any other conflicts. In the event that a player is suspended or expelled because of a discipline committee hearing there shall be NO REFUND. Only in the following events will a refund be granted, all other refund requests will be at the sole discretion of the Board.
1. The child cannot be placed upon a team due to lack of volunteers, coaches, or players prior to the start of the season.
2. The child cannot play due to a medical reason; a physician’s note must accompany the refund request.
3. The child moves out of the Burleson area.
4. At the sole discretion of the BISA Board of Directors.

Returned Check Policy |
| All checks returned to BISA as uncollected will be charged an additional $30 per check. Subsequent payments must be made with credit card or money order. Players will not be placed on a team until settled. |
